Risk Management Application

All Intramural & Travel Program coaches & assistant coaches, program managers & board members are required to complete the Risk Management Application before they can serve as volunteers to the club. Other types of volunteers (field helpers, Soccer Day helpers and team parents) are currently exempt from this requirement. In addition to the Risk Management Application, the Intramural Program asks its volunteer coaches to complete a Coaching Experience Form. The Travel Program requires its coaches to complete a Travel Coach Application. Both forms are found below:

Intramural Program – Coaching Experience Form

Travel Program – Coach Application

In order to serve as a Travel Program coach or assistant coach, please complete our Travel Coach Application – click here (all responses are kept confidential).

VOLUNTEER DISCLOSURE STATEMENT

Because the safety of our children is paramount, we require all volunteers participating as members of the coaching staff, program management or board of directors of Harleysville Soccer Association to complete the Risk Management Application before serving.

The Risk Management Application allows Harleysville Soccer Association, as a member of the Eastern Pennsylvania Youth Soccer Association (EPYSA) and U.S. Youth Soccer, to conduct a criminal history check. No financial information will be obtained.

All information contained within the criminal history check is kept strictly confidential and will only be available to members of Harleysville Soccer Association’s Disciplinary Board, which is currently comprised of its President, Vice-President and a joint-member of Harleysville Soccer Association and Lower Salford Township Police Department.

The Harleysville Soccer Association will deny a volunteer the opportunity to participate if he/she:

• Fails to submit the Risk Management Application or fails to do so in a timely manner; or
• Provides incorrect or misleading information within the Risk Management Application; or
• Has been convicted of a crime of violence or of a crime against a person.

Online submission of the Risk Management Application represents the volunteer’s consent to authorize the criminal history check.

The Risk Management Application needs to be submitted annually by each volunteer who wishes to continue his/her participation with Harleysville Soccer Association, even if information requested within the form has not changed from the prior year.
